Reporting a Test Result without photo (SSO Login)

1. Go to Care360 Dashboard

2. Type in your email address

3. Enter your SSO credentials

4. Click on Continue once you review any privacy policies.

5. Click on Report Test Result

6. Click on I Accept to accept the Data Disclosure statement

7. Click on the Test Result Dropdown and choose your result

8. Choose the date in the Test Date Dropdown

9. Choose your Test Type in the dropdown

10. Choose test from from Lab or Manufacturer Name (if not listed, select Other)

11. Click on Submit
