1. Open your User Profile and click Add COVID-19 Immunization Record
2. Review and accept the Data Consent Form
3. Select vaccine manufacturer
4. Enter vaccination details and click Submit
Note: Dates entered must match the dates on your vaccine record.
5. Your vaccination submission is complete. If it is your companies policy that vaccinations must be validated, your status will remain pending until validation by CareValidate is completed.
