Expired Health Assessment - You have not filled out your Health Assessment questionnaire in the timeframe required by your company. Log in and complete the Questionnaire.
Risk Factor- You have indicated during your Health Assessment Questionnaire that you have experienced a significant risk of exposure to COVID-19.
Symptoms- You have indicated during your Health Assessment Questionnaire that you are experiencing COVID-19 symptoms.
Positive Test - You have either submitted a positive test result, or you indicated during your Health Assessment Questionnaire that you have tested positive for the COVID-19 virus.
Daily Wellness Notification - If the option is enabled by your employer, there is also a Daily Wellness Notification email which reminds everyone to participate in the Health Assessment Questionnaire.
