1. Launch the Care360 app, and click 'Immunization' on the bottom ribbon
2. Read and accept the Data Consent form
3. Select the vaccine manufacturer from the dropdown list
4. Enter vaccination details (Vaccine proof may not be required based on your organization's policies and settings).
Note: Dates entered must match dates on your vaccine document
5. Your vaccination submission is complete. If your company requires Vaccine Validation, your status will be pending until validated by Care360
6. Clicking 'Done' will take you to your immunization summary as well as the option to upload booster information (booster requirements vary depending on organization requirements)
If you are notified of a failed vaccination verification or a failed vaccination document upload, you will receive information on the failure reason and possible vaccination reminders from Care360 depending on organization requirements.
